In Fiscal Years 2007 and 2008 (July 1, 2006 - June 30, 2008) you may be eligible to be reimbursed for your inpatient hospital co-pay ($250) and outpatient surgery co-pay ($150).
If you are enrolled in the City of Northampton's group health insurance, you will need to register for Member Self Service on Blue links (the Blue Cross/Blue Shield Website) or click on the logo below.
Once there, follow the instructions on the Co-Pay Reimbursement Instruction Page to complete your registration.
