If you are covering a dependent on the City's Group Health Insurance who is over the age of 19, you will need to fill out an affidavit annually. This verification paperwork is due back in the Human Resources office by October 24, 2008.

An affidavit is required for each enrolled dependent age 19 and over. If an affidavit is not received by the deadline, the child's participation in the health insurance will be ended as of October 31, 2008.

It is *critical* that we receive this information back by the stated deadline.
