In Fiscal Year 2010 (July 1, 2009 - June 30, 2010) you may be eligible to be reimbursed for your inpatient hospital co-pay ($250) and outpatient surgery co-pay ($150).
If you are enrolled in the City of Northampton's group health insurance, and you have services that are subject to these co-pays, you can request reimbursement by bringing the E.O.B. (Explanation of Benefits) that you receive from Health New England. EOB's will be mailed to your home address each time Health New England reviews/pays a bill on your behalf.
If you had services subject to these co-pays between 1/1/2009 and 6/30/2009 you will need to call Blue Cross/Blue Shield (Pat @800 393-1114 X 62920) and request a copy of the claim. You will need to know the Date of Service.
